Group Updates Type 2 Diabetes Care Guideline.
The American Association of Clinical Endocrinologists updated the group’s type 2 diabetes care guideline. The guideline, which was published online in conjunction with the American College of Endocrinology, adds a focus on lifestyle modifications and updates some of its information on diabetes medications. https://www.aace.com/sites/all/files/diabetes-algorithm-executive-summary.pdf

New Guidelines On Diagnosis and Treatment Of Primary Adrenal Insufficiency.
The Endocrine Society has issued a new Clinical Practice Guideline on diagnosis and treatment of primary adrenal insufficiency. The guidelines recommend immediate treatment of severe symptoms, even if diagnosis has yet to be confirmed. Stefan Bornstein, MD, who chairs the task force that put together the guidelines, said,

Risk of depression in Diabetes declines on second decade and then rapidly rises.
A study published in Maturitas reported the risk of depression following a diagnosis of diabetes falls during the second decade of having the disease and then rapidly rises, The study suggests there is a J-shaped relationship between the two conditions.Diabetes was associated with increased OR of ever (OR = 1.49, 95%CI = 1.25, 1.76) and current depression (OR = 1.94, 95%CI = 1.15, 2.48).

Research Suggests Link Between Thyroid Cancer And Breast Cancer.
Research suggests that the likelihood of thyroid cancer (TC) and breast cancer (BC) occurring in the same woman” may be greater than would occur by chance. Investigators found that 4.3% of TC patients subsequently developed BC, and 2.6% of BC patients later were found to have TC, both rates more than twice as high as the expected rates of each cancer in the general population. The findings were published in Thyroid.

Adrenal suppression from inhaled glucocorticoids is highly prevalent
Woods et al. carried out a retrospective analysis to determine the prevalence of adrenal suppression due to prescribed glucocorticoids and the utility of a morning serum cortisol for rapid assessment of adrenal reserve in the routine clinical setting.

FDA Approves First Pen Device With U-500 Insulin.
FDA approved the first prefilled pen containing concentrated U-500 strength insulin (Humulin R U-500 KwikPen 500 units/mL, Eli Lilly) for diabetes patients who require more than 200 units per day. The device is intended to be a “more convenient alternative to the current U-500 vial, which requires use of special syringe and dose conversions.

Treatment with Radiolabeled Peptide in Midgut Neuroendocrine Tumors.
The multicenter, phase III, international NETTER-1 trial presented at Gastrointestinal Cancer Symposium suggests individuals with previously treated midgut NETs had a 79% reduction in risk of disease progression or death when treated with a radiolabeled peptide. While the median progression-free survival (PFS) had yet to be reached an estimate placed the median at about 40 months for 177Lu-dotatate (Lutathera), versus 8.4 months for octreotide LAR. Preliminary survival analysis indicated nearly 50% fewer patients in the 177Lu-dotatate group died compared with octreotide LAR group.

Hyperbaric-Oxygen Therapy May Not Reduce Amputation For Diabetic Foot
A study in Diabetes Care reports that among 103 diabetic patients with a nonhealing foot ulcer who received standard care, those who additionally received hyperbaric-oxygen therapy were just as likely to need an amputation as patients who also received sham therapy, in a new randomized controlled trial. in addition, adjuvant hyperbaric-oxygen therapy did not improve wound healing compared with added sham therapy.
