הכנס השנתי עבר בהצלחה רבה, התוכנית הייתה מעניינת והארגון היה מופלא. ברצוננו להודות לפרופ’ דרק להרויט ו- פרופ’ יואב שרוני על השקעה בהרכבת תוכנית מדעית מצוינת. ברצוננו גם לברך את זוכי הפרסים לשנת 2014: ערן הורנשטיין (פרס לינדנר), הודרלנד דנה (פרס חוברס), אתי אושר (תקציר קליני מצטיין), בני גרפינקל (תקציר בסיסי מצטיין), עדי גיא, סרגיי רודניצקי ואהוד שרוני (פוסטרים מצטיינים).

חברי כבוד באגודה האנדוקרינית

בכנס השנתי חולקו לראשונה אותות הוקרה “חבר כבוד” לחברי האגודה על תרומתם לאנדוקרינולוגיה בישראל. הזוכים השנה הם: פורפ’ יצחק קוך, פרופ’ מנחם שפירא, פרופ’ שמעון גליק, פרופ’ זקי קריים, פרופ’ צבי לרון

ב 30/04/14 התקיים ברמת גן כנס Bone 2014. זו השנה השנייה ברציפות שיו”ר הכנס, פרופ’ צופיה איש שלום בונה תוכנית המשלבת עדכון ונושאים שבמחלוקת בתחום. השנה, פרופ’ ליברמן דיבר על תרופות לטיפול באוסטיאופורוזיס שבדרך, ביניהן מעכב catepsin K ונוגדנים ל sclerostin. נידונו סוגיות שבדרך כלל מקבלות חשיפה מעטה: בניית שלד תקין עוד בשלב העוברי, לדוגמה הקשר בין פרמטרים שלדיים בעובר למצב ויטמין די אצל האם. פרופ’ קונסטנטיני הרצתה על השפעת פעילות גופנית נמרצת על מבנה עצם אצל ילדים ומתבגרים. התקיים debate על מיקומה של בדיקת צפיפות עצם באבחון ומעקב חולה אוסטאופורוטי. דווח כי חלה התקדמות משמעותית בבניית database ישראלי כחלק ממדד ה FRAX. פרופ’ ליאל ודר’ פרנקל התייחסו בהרצאותיהם לטיפול מגן עצם בנשים סמוך לגיל חדלון הווסת. נידונו סוגיות באבחון וטיפול שברים עם ההשפעה הגבוהה ביותר על תחלואה ותמותה של חולי אוסטיאופורוזיס – שברי חוליות וירך. ובנוסף נסקר גם נושא מניעת נפילות. FRAX Underestimates Incident and Recurrent Fractures
A study in JCEM reported high-risk FRAX score present in only 42.7% patients before and 56.4% after the incident Fx. Most Fx patients at low or moderate risk before initial Fx were men, younger than 65 yrs, or without previous Fx. Over 3 years follow-up, recurrent Fx occurred in 108 patients (2.7 per 100 patient-years). The sensitivity of post-Fx FRAX to predict a recurrent Fx was 71.3% and was specifically lower in patients younger than 65 years (13%) and without previous Fx (63%) at inclusion.

Firms Ordered To Pay $9 Billion For Hiding Cancer Risks Of Diabetes Medicine.
Takeda Pharmaceutical Co. and Eli Lilly & Co. were ordered to pay a combined $9 billion in punitive damages after a jury in a first such federal trial in U.S. found they concealed the cancer risks of their Actos. Takeda was ordered to pay $6 billion while its partner Lilly was asked to pay the rest

FDA Panel Recommends Approval For Inhaled Form Of Insulin.
An outside FDA panel recommended approval of a new inhaled insulin, Afrezza. The panel voted 13 to 1 that Afrezza was safe and effective enough for approval as a treatment for T1DM. According to the article, the endorsement for T2DM, where insulin is usually one treatment option among many, was unanimous. MannKind last August disclosed that the inhaled form of insulin for T2DM in conjunction with oral medication performed significantly better than an oral therapy alone in a Phase 3 study. On March 28 however, the FDA raised concerns that the treatment may affect lung function.

Low Vitamin D Linked To Higher Risk Of Death From Cancer, Heart Disease.
Two studies in BMJ suggest individuals with low vitamin D are more likely to die from cancer and heart disease and to suffer from other illnesses. In one study, researchers found that adults with lower vitamin D had a 35 percent increased risk of death from heart disease, 14 percent greater likelihood of death from cancer, and a greater mortality risk overall. The other study suggested that high D levels may potentially help prevent conditions, such as diabetes, but researchers said there was no highly convincing evidence that vitamin D affected any of the outcomes they examined.

Diabetes May Be Linked To Higher Risk Of Pancreatic Cancer.
A research published online in Annals of Surgical Oncology suggests individuals with diabetes have double risk of pancreatic cancer. Investigators came to this conclusion after analyzing data from 88 studies. Still, experts note that the overall risk remains low.

Kisspeptin 1 Protein May Contribute To Development Of T2DM In Mice.
A study in Cell Metabolism suggests kisspeptin1, a protein made in the liver known to regulates fertility hormones, suppresses insulin production and contributes to develop T2DM in mice. The protein upregulated by glucagon, is often elevated in early T2DM via camp-PKA-CREB signaling. A trihormonal regulatory circuit between pancreatic alpha cells, hepatocytes, and beta cells, and assign kispeptin1 an unexpected role in liver-to-islet endocrine signaling has been suggested.

AACE/ACE State on Molecular Diagnostic of Indeterminate Thyroid Nodules
Approximately 10 to 25% of thyroid FNA yield an indeterminate result. The risk of malignancy varies between 15 and 30% for this category. Two principal tests are currently marketed to improve the malignancy risk assessmen. “Rule In” and “Rule Out” tests attempt to confirm or exclude the presence of cancer within a thyroid nodule by means of robust PPV or NPV, respectively. The Rule In tests determine the presence of single gene point mutations (BRAFV600E or RAS) or gene rearrangements (RET/PTC, PAX8/PPARγ) shown to predict cancer, while the Rule Out test (Afirma® gene expression classifier, GEC) utilizes a gene expression classifier (RNA expression) designed to define a process as benign. Among the presently available tests, only the BRAFV600E and RET/PTC rearrangement are associated with a PPV that approaches 100%. The category indeterminate nodule, cytopathology practice patterns, and the prevalence of malignancy within the population tested all impact the NPVs and PPVs for the tests in question. At present, molecular testing is meant to complement and not replace clinical judgment, sonographic assessment, and cytopathology interpretation.

Endocrinologists Rank Towards Bottom Of Medical Specialties Compensation.
Endocrinologists ןin the USA rank towards the bottom of medical specialties when it comes to annual compensation, according to Medscape’s 2014 Physician Compensation Report. Endocrinologists’ average annual earnings in this year’s survey came in at $184,000, placing them barely ahead of pediatrics, family medicine, and HIV/infectious disease. While endocrinologists were less satisfied with their income than the average physician (41% vs 50% income satisfaction), other measures of job satisfaction revealed that most endocrinologists are happy to be physicians: 57% reported that, if they had to do it again, they would still choose to practice medicine.

Researchers Use Cloning Technique To Make Insulin-Producing Cells
Research published in Nature is a step toward providing genetically matched replacement cells for transplant, said study leader Dieter Egli of the New York Stem Cell Foundation Institute. Researchers took skin cells from a woman suffering from type 1 diabetes, reprogrammed them into embryonic stem cells, and then converted those cells into insulin-producing cells in mice. This is the second report this month of success in using cloning technology to make human embryonic stem cells

Shoulder of Giants

Frederick Sanger, (13 August 1918 – 19 November 2013) was a British biochemist who won the Nobel Prize for Chemistry twice, the fourth person overall with two Nobel Prizes. He was born in Rendcomb, a small village in Gloucestershire, England. In 1936 moved as an undergraduate to St John’s College, Cambridge to study natural sciences. He was a pacifist and a member of the Peace Pledge Union. It was through his involvement with the Cambridge Scientists’ Anti-War Group that he met his wife, Joan Howe. He graduated in December 1940. In 1943 was awarded a doctorate for his thesis “The metabolism of the amino acid lysine in the animal body”. He then joined the group of Charles Chibnall, chair of the Department of Biochemistry who had already done some work on the amino acid composition of bovine insulin. Sanger looked at the amino groups in the insulin. Insulin was one of the very few proteins that could be purchased in its pure form. Sanger’s first triumph was to determine the complete amino acid sequence of the two polypeptide chains of bovine insulin, A and B, in 1951 and 1952, respectively, His principal conclusion was that the two polypeptide chains of insulin had precise amino acid sequences and, by extension, that every protein had a unique sequence. This achievement earned him his first Nobel prize in Chemistry in 1958. The discovery was crucial for the later sequence hypothesis of Crick for developing ideas of how DNA codes for proteins. Sanger was a member of the Medical Research Council and when they opened the Laboratory of Molecular Biology in 1962, he became head of the Protein Chemistry division. Soon after his move he started looking at the possibility of sequencing RNA molecules and began developing methods for separating ribonucleotide fragments generated with specific nucleases. In 1977 Sanger and colleagues introduced the “dideoxy” chain-termination method for sequencing DNA molecules, also known as the “Sanger method”. This was a major breakthrough that earned him his second Nobel prize in Chemistry in 1980. The dideoxy method was eventually used to sequence the entire human genome. And this is probably what Isaac Newton meant in his above quote, “shoulder of giants”.
